Rullamas-Valencia3 aMethods: Convenience sampling was done. Among the 100 patients who gave their consent, 52 patients completed the study. Data were processed using SPSS and analyzed using ANOVA and Pearson correlations. Results: Results showed that the ACD video presentation affected the respondent's perception on their health status, perception on the consequences of this status, medical treatment plan, knowledge on ACD, willingness to have ACD, choice of surrogate decision makers, and a significant effect on the preferred type of medical treatment in terminal cases. ANOVA and Pearson correlations showed that age, gender, ethnicity, education, medical history and religion had influence on the knowledge and attitudes of respondents. Results showed that senior citizens mostly with chronic illness have a borderline knowledge of the consequences of their illness. ACD video presentation produced a heightened awareness of their health conditions and the type of medical treatments in end of life situations. Comfort care and limited type of care are the popular choices over doing everything in terminal cases. No significant change was noted on the respondent's choice, pre and post intervention, of surrogate decision makers. Majority chose their children, spouses, attending physicians, and siblings, in that order. Conclusion: Most respondents think that Advance Care Directives should be part of the medical record and be done in hospitals. Secondly, distribution to senior citizens organizations should be done.
