    <subfield code="a">OBJECTIVE: To conduct a baseline study on the prevalence, diagnosis and management of chest X-ray (CXR) positives among adolescents in a school setting

DESIGN: Descriptive, population-based prevalence study.

SETTING: St Pauls College Manila.

SUBJECTS: All college students enrolled for the school year 1997-1998 who have been found to have chest X-ray findings of pulmonary tuberculosis (PTB) using purposive sampling.

MAIN OUTCOME MEASURES: 1) Baseline prevalence of PTB in the population studied, 2) description of the medical interventions performed in terms of diagnostic modality used and of treatment. 3) compliance rate to the intervention performed, 4) description of the outcomes in terms of treatment completion and follow-up chest X-ray findings, and 5) identification of health resources utilized for medical diagnosis and treatment.

RESULTS: There were 3,041 students enrolled and 94 percent had chest X-ray. Thirty-one X-ray films were interpreted as (+) for PTB. The prevalence is 31/3, 041 or 1 percent which is lower than the national prevalence of 4.7 percent of CXR positives among asymptomatic patients. Twenty-two students were followed up for one year. Only six of the students complied with the recommendation for sick leave. Majority of the students were not able to comply with the recommended sputum culture, although roughly half were able to comply with sputum AFB smear. Sputum AFB smear positive was similar to the national prevalence of 0.3/1,000. Majority of the students were treated by specialists who were mainly hospital-based. Eighteen students were able to complete at least six months of treatment with 15 undergoing follow-up CXR. Thirteen of these chest films showed improvement during or after treatment.

CONCLUSION: The prevalence of PTB among the participants was lower than the national figure. However, incidence of sputum AFB smear positive was similar to national prevalence. Chest X-ray while not diagnostic of PTB is still useful. The PPD (purified protein derivative) as a screening tool should be made more readily available. Good compliance can be achieved through follow-up monitoring strategies.
