| Summary, etc. | The objective of this study was to establish a clinical profile of gout among patients admitted to the Cebu Doctors Hospital Department of Medicine. Enrolled in the study were 88 of 100 patient with a discharge diagnosis of gout and has met the criteria for acute gout by Wallace et al (1977). A review was made based on the following: age at onset & sex distribution, initial & subsequent join involvement, associated illnesses and precipitating factors, incidence of tophi and urinary stone, laboratory parameters and treatment undertaken. There were 76 males and 12 females with a ratio of 6.3:1 Mean age of onset for males was 57.8 while 62.4 for females. The first metatarsophalageal joint was predominantly involved was seen initially in 25 (28.4 percent) of patients. Oligoarticular involvement was seen initially in 9 (10.2 percent) of patients. Subsequent joint involvement was seen in 33 of 88 patients primarily affecting the knees in 17 (44.8 percent) patients. Hypertension, diabetes mellitus and urate nephropathy were the major illnesses noted in this series. Incidence of tophi and urolith were 6.8 percent and 21.6 percent respectively mean uric acid level was 8.1 mg percent. Mean creatinine was 1.7 mg percent. Rheumatoid factor were negative in all patients tested for it. Colchicine & NSAID were the initial medications given. In conclusion gout still predominantly involved males with an earlier age of onset. Initial arthritic attack is mainly podagra, subsequently affecting the knees. Hypertension is the most common associated medical illness. Identification of monosodium urate crystals in the synovial fluid were made in 29 to 31 patients. It serves as a sine qua non for the diagnosis of gout. |
